• Please visit our PATIENT PORTAL to request an appointment, send your doctor a message or view your recent medical record.
• You can also call the office to schedule or change an appointment.
• If you call our office and need to leave a voice-mail message, please be sure to speak clearly. Leave your name, date of birth, a phone number for us to call you back, and reason for your call. Our goal is to return phone calls on the same business day.
• If you need a prescription refilled, please call your pharmacy. If there are no refills available, the pharmacy will contact our office. ALSO NOTE that some prescriptions require prior authorization by your insurance plan which may take several days to obtain. Please plan accordingly and request refills at least 3-5 days before running out of your medication. Our office does not handle prescription inquiries after hours, over the weekend, or on holidays.
In the event that your doctor is not available, the Emergency Department at Colorado Canyons Hospital & Medical Center is available for emergent visits.
